Sierra's story: how a ketogenic diet transformed a dog's cancer journey
This is Jan's story, told in her own words. It's about her black Labrador Sierra, a diagnosis that felt like a death sentence, and a decision that changed everything.
"In late 2018, our sweet black lab Sierra started limping. She underwent x-rays, and our vet confirmed the bad news: she had osteosarcoma in her right humerus. Our veterinarian gave her 3–5 weeks to live. However, if we went through with amputation, chemo, and radiation, we could possibly hope to keep her alive for a year.
We chose to forego treatment and preserve Sierra's quality of life. We found KetoPet online and quickly transitioned to a ketogenic diet. And the results were AMAZING.
Six months later, Sierra was still alive and well. However, her limp was getting worse. We decided to do the amputation for pain management — and we're glad we did. We thought she was a happy lab before surgery — but after the procedure — we could tell she was much happier. The same veterinarian who expected her to live 3–5 weeks was SHOCKED to see her alive, and even MORE shocked to see her so happy in her pre-op visit.
While her amputation showed that she had the most aggressive form of osteosarcoma, her pre-op chest x-ray and organ ultrasounds did not show ANY signs of metastasis. Our veterinarian was truly amazed.
Sierra recovered quickly from her surgery and is thriving on her ketogenic diet. She gleams with pride every time she has a new accomplishment, and her success remains so rewarding to us. Her happiness is contagious and we're so grateful to have her around."

What is osteosarcoma?
Osteosarcoma is one of the most aggressive forms of bone cancer in dogs, particularly common in larger breeds. It spreads rapidly and carries a poor prognosis — the standard treatment protocol of amputation combined with chemotherapy typically extends survival by months, not years. A 3–5 week prognosis without treatment is not unusual.
Sierra's outcome — alive and thriving six months later with no signs of metastasis — is remarkable by any measure.
The role of the ketogenic diet
The scientific basis for using a ketogenic diet as adjunctive cancer therapy is grounded in the Warburg Effect — the observation that cancer cells are almost entirely dependent on glucose for energy, consuming it at rates far higher than healthy cells. A ketogenic diet dramatically reduces circulating glucose, effectively cutting off the primary fuel supply that cancer cells depend on.
This is the science that KetoPet Sanctuary spent seven years and $7 million investigating — rescuing dogs with terminal cancer from kill shelters and studying what happened when they ate a very low-carbohydrate, high-fat diet. Sierra's story is consistent with what KetoPet observed across dozens of dogs: that a ketogenic diet, used alongside appropriate veterinary care, can meaningfully change outcomes for dogs with cancer.
Sierra's ketogenic diet was built around fresh muscle meat, fatty fish, and organ meat as the base — with eggs, healthy oils and leafy green vegetables. Very low in carbohydrates. High in quality animal fat and protein. Designed to shift her metabolism away from glucose dependency and toward fat-based energy production.
A note on individual outcomes
Sierra's story is inspiring — and it's real. But every dog and every cancer is different. A ketogenic diet is not a cure, and it is not a replacement for veterinary care. What it can be is a powerful adjunctive therapy that supports the body's own defences while making the metabolic environment less hospitable to cancer cells.
If your dog has been diagnosed with cancer and you're considering a ketogenic diet, the most important first step is an open conversation with your veterinarian. Bring the research. Ask questions. Make decisions together.
Sierra's owner Jan, did exactly that — and gave her dog a story worth telling.
